Requirements for a New Born Baby

1. Two application forms completed and signed by the parents (Click here)
2. Three passport size photographs
3. A copy of the father’s Kuwait passport
4. A copy of the mother’s passport
5. The original Birth certificate of the child
6. Birth certificate (original long form) has to be authenticated by the Canadian Ministry of Foreign Affairs.
7. A copy of the marriage Certificate
8. $60 Canadian dollars for issuing an Emergency Travel Document
9. $30 Canadian dollars for legalization of birth certificate
10. You can issue one money order for $90.00 Canadian dollars for both documents.
11. Money order payable to the Embassy of Kuwait
12. Prepaid FedEx waybill, Purolator, UPS, DHL or Express post. Envelope and Waybill must be enclosed inside for return. Clearly print your name, address, phone number and courier account number for return.
