Family to follow Visa (Canadian passport holders)

1. Original CanadianPassport with a minimum of six month validity.
   
   
2. Original Passport with a minimum of six month validity, ( please note none Canadian Passport holders can process their Employment Visa through our office. An EXTRA $30.00 canadian dollars fee is requested. A form must be filled and signed by the applicant that will be provided upon request.

4. One passport size photograph
   
   
5. ENTRY FAMILY TO FOLLOW VISA one paper issued from the Ministry of Interior Immigration department in Kuwait will be sent to applicant from employer in Kuwait prior to applying for a visa.
   
   
6. All applicants working in Kuwait must have a Complete Physical Examination for Contagious and Disabling Disease. Can be obtained from any doctor in Canada.
   
   
7. HIV virus and Viral Hepatitis B and C copy of the laboratory test must be attached).
   
   
8. A fee $60 Canadian fund. The fee must be paid by cash, money order or certified cheque.
   
   
9. Police Clearance Certificate Required from your local police station must be obtained:
• Original Police clearance has to be stamped by Ministry of Foreign Affairs and International Trade. Department of Foreign Affairs (JLAC) - 125 Sussex Drive, Ottawa, ON K1A 0G2.

• Prepaid FedEx waybill or Purolator, UPS, DHL or Express post. Envelope must be enclosed inside for return. Clearly print your name, address, phone number and courier account number for return
  
  
• If lodging documents in person to the Embassy this should be done between the hours 9:00a.m. and 12:30p.m.
  
  
• Customers will be contacted once the documents are ready for pickup
  
  
• Criminal Record Checks, Fingerprint/Clearance Certificates
  
  
10. As of February 1st, 2010 all criminal record checks, fingerprint/clearance certificates, and any documents orginating from the RCMP, provincial or local police stations must first be notarized prior to authentication
    
    
11. You will be contacted once the passport is ready for pick up
